Every year ALL THDC athletes must ensure they have completed each form on this checklist:

• Complete AAU Number Annual Confirmation form (it expires annually in August) - see New Member Information page for 'How to' instructions

• Complete read Handbook, then Acknowledgment of Handbook form

• Complete Code of Conduct form

• Complete Waiver & Release of Liability form

• Complete Medical & Consent for Treatment form

• Complete THDC Media Release form

• Complete Contact Information Release form

• Complete ImPACT Baseline Test - ages 12+ and submitted results to thillsdiveclub@gmail.com *please note - $20 fee for testing*

Additional item to be completed:

• Complete Quarterly Commitment form (Sept - Nov; Dec - Feb; Mar - May; Jun - Aug)
